when you create a superflat world, and set the biome to mesa or ;38; the mineshafts generate inside the bedrock layer, possibly making dungeons go through bedrock when they spawn.
steps to reproduce: make a superflat world, set it to "overworld" and set the biome from ;1; to ;38; and remove the stone layer, set the seed to 5 then use the command tp 669 1 -151 and you will spawn in a glitched mineshaft. if you make the ground level higher, the mines may rarely spawn groundless dungeons. also igloos have a chance of going through the bedrock layer.
to reproduce with igloos, make the ground lower in the "snowy kingdom" preset. anyone that can fix this can put more restrictions to the bedrock layer that makes the final bedrock layer "generated structure proof".
