When using efficiency 3 4 and 5 tools, blocks broken too quickly will sometimes become invisible.

For example using an efficiency 4 diamond shovel on dirt will cause some dirt blocks to become invisible. This occurs in single-player worlds. These blocks can no longer be interacted with until the world is reloaded.

This happens with a frequency of one bugged block every 192 blocks per estimated average.
